| These forms can be filled out on line, downloaded to a computer and
filled out electronically, or printed and filled out manually. Many require signatures
so they will need to be printed using the print button at the top of the form. Obtain the
appropriate signatures and mail to the address on the form. If signatures are not
necessary, there will be an email button also at the top of the form.

- APPLICATION FOR INDIVIDUAL MEMBERSHIP
All members will be Daughters-at-Large until their Chapters are instituted. When you fill
out the individual member registration form you will be asked to choose the size of
cross you want sent to you. The larger cross is 1.5 inches tall and 1.25 inches across.
